Miliary retinitis in coccidioidomycosis.

B J Glasgow, H H Brown, R Y Foos

    American Journal of Ophthalmology
    |July 15, 1987
    PubMed
    Summary

This study details a rare case of coccidioidomycosis retinitis in a child, revealing numerous retinal and choroidal granulomas. The ocular lesions were randomly distributed in the posterior fundus, offering insights into this fungal infection of the eye.

    Area of Science:

    • Ophthalmology
    • Mycology
    • Pathology

    Background:

    • Coccidioidomycosis is a fungal infection endemic to the southwestern United States.
    • Retinitis, an inflammation of the retina, can be a severe manifestation of disseminated fungal infections.
    • This study examines a rare pediatric case of coccidioidomycosis retinitis.

    Observation:

    • Autopsy revealed multiple small lesions in vital organs.
    • Ocular lesions were confined to the posterior fundus.
    • Detailed analysis identified 29 retinal and 22 choroidal granulomas in both eyes.

    Findings:

    • Lesions were randomly distributed throughout the fundus.
    • Retinal granulomas were located in layers supplied by retinal circulation.
    • Choroidal lesions primarily affected the middle vascular layer, with minimal disruption of Bruch's membrane and intact pigment epithelium.
    • Retinal and choroidal granulomas showed no topographic association.

    Implications:

    • Provides detailed pathologic insights into a rare form of fungal retinitis in pediatric patients.
    • Highlights the importance of considering disseminated fungal infections in ocular presentations.
    • Contributes to understanding the specific patterns of ocular involvement in coccidioidomycosis.
