An international study on activated partial thromboplastin time prolongation. Part 2: Interpretative commenting

Interpretative comments from clinical labs are essential for guiding physician decisions. Coagulation tests like APTT are particularly important as they may signal deeper issues requiring specialized follow-up. Prior research has shown that these comments can influence diagnostic accuracy and treatment planning. However, the quality of such comments remains understudied. This gap motivated a survey of how labs interpret APTT results. No prior work had resolved how often these comments are accurate or misleading. The study aimed to address this uncertainty by evaluating ICs in a controlled setting. It was already known that APTT prolongation can arise from multiple causes, including heparin contamination or factor deficiencies. That uncertainty drove the need to assess how labs differentiate these scenarios in their reports.

The study aimed to evaluate the quality of interpretative comments provided by clinical laboratories for APTT results. It focused specifically on two scenarios: heparin contamination and factor VIII deficiency. The motivation was to determine how consistently and accurately labs interpret abnormal coagulation results. The researchers sought to assess the clarity, accuracy, and usefulness of these comments. The goal was to identify strengths and weaknesses in current practices. The study also aimed to determine how often labs address the clinical question posed in the case. The study used two lyophilized plasma samples with known abnormalities. Each sample was paired with a fictional clinical case scenario. Laboratories were asked to perform APTT and APTT mixing tests, then provide interpretative comments. The samples were distributed to 214 European labs. The ICs were evaluated based on several criteria: analytical classification, interpretation, advice to physicians, clarity, length, and relevance. Each comment was scored for accuracy and helpfulness. The evaluation focused on whether labs correctly identified the cause of APTT prolongation. The researchers also assessed whether the advice was appropriate for the clinical context. This approach allowed for a standardized assessment of IC quality across multiple labs.

A total of 214 laboratories participated in the study. Analytical classification was provided in 57% of comments. For case 1, 91% of labs provided an interpretation, with 9.3% being incorrect. For case 2, 83.3% of labs interpreted the result, with 6.5% errors. Advice was given in 65.8% of case 1 comments and 61.2% of case 2 comments. Of these, 36% and 4.7% were considered incorrect, respectively. More than 70% of comments were clear and of appropriate length. The majority of labs provided useful interpretations and advice. However, a significant number of comments were found to be confusing or misleading. These findings suggest variability in the quality of ICs across laboratories.
