Summary

This study identified four circulating microRNAs (miRNAs) associated with relapse risk in major depressive disorder (MDD). These findings may help predict recurrence and guide early intervention for patients with MDD.

Background:

  • Major depressive disorder (MDD) is characterized by recurrent episodes, necessitating biomarkers to predict relapse.
  • Identifying individuals at high risk for recurrence is crucial for timely intervention and improved patient outcomes.

Purpose of the Study:

  • To investigate the association between circulating microRNAs (miRNAs) and relapse risk in patients with recurrent major depressive disorder.
  • To explore potential miRNA biomarkers for predicting future depressive episodes.

Main Methods:

  • Serum samples from 217 participants (63 relapsers, 154 non-relapsers) in the OBSERVEMDD0001 study were analyzed for 575 circulating miRNAs using the ID3EAL™ miRNA Discovery Platform.
  • miRNA levels were quantified using patented qPCR technology and normalized with spike-in controls.
  • Linear models adjusted for age, gender, and plate were used to test the association between miRNAs and relapse status.

Main Results:

  • Four circulating miRNAs showed nominal association with relapse status (false discovery rate adjusted p-value < 0.1).
  • Enrichment analysis identified 112 significantly enriched pathways, including neurogenesis, cytokine response, and neurotrophin signaling.
  • These pathways are implicated in the pathophysiology of major depressive disorder.

Conclusions:

  • Specific circulating miRNAs may serve as potential biomarkers for predicting relapse in major depressive disorder.
  • These miRNAs could regulate genes involved in key signaling pathways relevant to MDD.
  • Further validation in larger, independent cohorts is needed to develop a predictive miRNA signature for early intervention.
