Background:

  • Osteosarcoma (OS) is the most common primary bone cancer.
  • Choroidal metastasis is the most frequent intraocular tumor in adults, but rarely linked to sarcomas.
  • Only two prior cases of OS-related choroidal metastasis exist, both reported before 1970.

Observation:

  • A 20-year-old male with tibial OS, amputation, and metastases presented with decreased vision.
  • Ophthalmic exam revealed a large subretinal mass in the posterior pole of the right eye.
  • Imaging studies (B-scan, CT, MRI) characterized the mass, noting a height-to-base ratio (HBR) of 1-1.2 and specific signal intensities.

Findings:

  • Choroidal osteosarcoma metastasis can manifest as a pink nodule.
  • High HBR and intralesional hyperreflective deposits are characteristic features.
  • The lesion showed hyperdensity and contrast enhancement on CT, and specific signal characteristics on MRI.

Implications:

  • Sudden visual changes in patients with metastatic OS warrant close ophthalmologic and oncologic evaluation.
  • This case highlights the importance of multidisciplinary collaboration in managing rare metastatic presentations of osteosarcoma.
  • Prompt diagnosis and management are essential for preserving vision in patients with systemic metastatic disease.
