    A rare case report details a huge sacrococcygeal teratoma successfully removed surgically in a 29-year-old male. This teratoma was uniquely associated with sacral agenesis, a combination not previously documented in medical literature.

    Background:

    • Sacrococcygeal teratomas are congenital tumors with varying degrees of malignancy.

    • The patient's cystometry and myelography results were within normal limits.
    • A comprehensive literature review did not identify any previously reported cases associating sacrococcygeal teratoma with sacral agenesis.
    • Embryological factors potentially contributing to this rare association were discussed.

    • This case highlights the importance of thorough imaging and evaluation for associated anomalies in patients with sacrococcygeal teratomas.
    • Understanding the embryogenesis of these co-occurring conditions may improve diagnostic accuracy and treatment strategies.
