Real-time reverse transcription-polymerase chain reaction, or Real-time RT-PCR, is an analytical tool used to determine the expression level of target genes. The method involves converting mRNA to complementary DNA with the help of an enzyme known as reverse transcriptase, followed by the PCR amplification of the cDNA. Summary

The ADVIA Centaur SARS-CoV-2 antigen (COV2Ag) assay shows high agreement with nucleic acid amplification tests (NAAT), meeting WHO performance standards. This high-throughput COV2Ag test is suitable for large-scale screening programs.

Background:

  • Independent assessment of SARS-CoV-2 antigen (COV2Ag) tests is crucial due to performance variability.

Purpose of the Study:

  • To assess the performance of a new high-throughput COV2Ag test.
  • To compare the COV2Ag assay against SARS-CoV-2 nucleic acid amplification tests (NAAT).

Main Methods:

  • 347 nasopharyngeal samples were tested using NAAT (CDC LDT or GeneXpert) and the ADVIA Centaur COV2Ag assay.
  • Samples were collected between January and October 2021.

  • Overall agreement between COV2Ag and NAAT was 85.6% (82.4% for NAAT-positive, 97.3% for NAAT-negative specimens).
  • Agreement was higher in symptomatic individuals (90.0%) compared to asymptomatic individuals (62.5%).
  • Agreement improved with lower cycle threshold (Ct) values in NAAT-positive samples.

Conclusions:

  • The COV2Ag assay performance exceeded WHO minimum requirements (≥80% sensitivity, ≥97% specificity).
  • The assay's high-throughput nature and reduced wait times make it valuable for large-scale screening.
