Background:

  • Hypertension is increasingly recognized as a significant comorbidity in cancer patients.
  • Limited data exist on the association between blood pressure (BP) levels and incident heart failure (HF) or cardiovascular disease (CVD) events in this population.

Purpose of the Study:

  • To examine the association between BP classification, according to the 2017 ACC/AHA guidelines, and the risk of HF and other CVD events in patients with cancer.
  • To identify specific BP thresholds linked to increased cardiovascular risk in cancer patients.

Main Methods:

  • A cohort of 33,991 patients with a history of breast, colorectal, or stomach cancer was studied.
  • Patients on BP-lowering medications or with prior CVD were excluded. BP was categorized as normal, elevated, stage 1 hypertension, or stage 2 hypertension.
  • The primary outcome was incident HF, with follow-up for a mean of 2.6 years.

Main Results:

  • Over 779 HF events occurred. Elevated BP, stage 1 hypertension, and stage 2 hypertension were associated with increased HF risk (HRs 1.15, 1.24, and 1.99, respectively) after adjustment.
  • A stepwise increase in risk was observed for other CVD events as well.
  • These associations persisted even in patients undergoing active cancer treatment and were confirmed in a separate database.

Conclusions:

  • Medication-naïve stage 1 and 2 hypertension are linked to a higher risk of HF and CVD events in cancer patients.
  • Multidisciplinary collaboration between oncologists and cardiologists is essential for optimal hypertension management in this vulnerable group.
