Summary

Short-time-window myocardial Ki imaging using a population-based arterial input function (IF) and Bayesian penalized likelihood (BPL) reconstruction shows high correlation with standard long-time-window methods. This approach offers a practical alternative for cardiac sarcoidosis assessment.

Background:

  • Myocardial Ki imaging assesses cardiac sarcoidosis activity.
  • Standard methods use long imaging windows and patient-specific arterial input functions (IF).
  • Short-time-window imaging with population-based IF and Bayesian penalized likelihood (BPL) reconstruction is explored as an alternative.

Purpose of the Study:

  • To evaluate the feasibility of short-time-window myocardial Ki imaging.
  • To compare it with long-time-window imaging using patient-specific IF.
  • To assess the utility of population-based IF and BPL reconstruction.

Main Methods:

  • 18F-FDG-PET/CT scans from 30 cardiac sarcoidosis patients were analyzed.
  • Ki images were generated using individual patient IF (long window) and population-based IF with BPL (short window, varying beta values).
  • Image quality and Ki parameters (Ki-max, Ki-mean, Ki-volume) were assessed and compared.

Main Results:

  • Short-time-window images with population-based IF and BPL reconstruction (beta=1000) showed high visual quality.
  • High coefficients of determination were found between short-window (beta=1000) and long-window Ki parameters (0.91-0.92 for Ki-max/mean, 0.75-0.60 for Ki-volume).
  • Population-based IF with BPL reconstruction demonstrated strong correlation with individual patient-based IF methods.

Conclusions:

  • Short-time-window Ki imaging with population-based IF and BPL reconstruction is feasible.
  • This method closely correlates with traditional long-time-window imaging.
  • It presents a practical alternative for myocardial Ki imaging in cardiac sarcoidosis.
