    Background:

    • Assessing consciousness and predicting recovery in patients with Disorders of Consciousness (DoC) is complex.
    • Current methods rely on the Coma Recovery Scale-Revised (CRS-R), but prognostic accuracy needs improvement.

    Purpose of the Study:

    • To develop a novel, data-driven index for assessing consciousness domains using CRS-R sub-items.
    • To evaluate the prognostic power of this new index compared to existing clinical assessments.

    • A clustering algorithm was applied to the six sub-items of the CRS-R from a dataset of 190 DoC patients.
    • The resulting index, Consciousness-Domains Index (CDI), was compared with the Clinical State at Admission (CSA) and neurological outcomes at six months.

    • A two-cluster model was optimal, with visual and motor sub-items being most influential.
