Gas-containing mesenteric desmoid-type fibromatosis: A case report

A rare gas-containing mesenteric desmoid-type fibromatosis was diagnosed in a patient presenting with abdominal pain. Surgical resection was successful, with no recurrence after six months, highlighting the need for advanced imaging in diagnosis.

Background:
- Desmoid-type fibromatosis is a rare benign neoplasm, with only 8% occurring intra-abdominally.
- Mesenteric involvement is infrequent, necessitating differentiation from gastrointestinal stromal tumors, especially with gas-containing masses.
Observation:
- A 69-year-old male presented with hematochezia and abdominal pain.
- CT revealed a 3.9x3.6 cm gas-containing mass in the duodenum, exhibiting heterogeneous enhancement and cystic/air bubble components.
Findings:
- Postoperative pathology confirmed mesenteric desmoid-type fibromatosis.
- Surgical resection of the intra-abdominal lesion was performed.
- No local recurrence was observed at 6-month follow-up.
Implications:
- Preoperative diagnosis of intra-abdominal gas-containing masses is challenging.
- Spiculated infiltrative margins on imaging may suggest desmoid-type fibromatosis.
- Further research on imaging characteristics and treatment strategies is warranted.
