    Summary

Two-dimensional echocardiography (2DE) significantly enhances endomyocardial biopsy (EMB) procedures by accurately visualizing the bioptome tip, enabling targeted sampling, and predicting successful tissue acquisition.

    Area of Science:

    • Cardiology
    • Medical Imaging
    • Interventional Procedures

    Background:

    • Endomyocardial biopsy (EMB) is crucial for diagnosing cardiac conditions.
    • Accurate targeting of the biopsy site is essential for diagnostic yield.
    • Fluoroscopic control alone has limitations in visualizing the bioptome tip precisely.

    Purpose of the Study:

    • To evaluate the utility of two-dimensional echocardiography (2DE) in guiding endomyocardial biopsy (EMB).
    • To assess 2DE's capability in identifying the biopsy site, enabling selective sampling, and predicting positive tissue acquisition.

    Main Methods:

    • Conducted 62 EMB procedures in 53 patients with concurrent 2DE and fluoroscopic monitoring.
    • Compared echocardiographic visualization of bioptome tip contact with the ventricular wall to actual tissue presence.
    • Prospectively assessed 2DE's ability to predict positive sampling in 29 biopsies.

    Main Results:

    • 2DE visualized bioptome tip contact with the endocardium in 86% of right ventricular and 85% of left ventricular biopsies.
    • 2DE monitoring facilitated site selection and successful sampling from different ventricular areas.
    • 2DE predictions of positive sampling were confirmed in 93% of right and 100% of left ventricular samples.

    Conclusions:

    • 2DE monitoring provides accurate forceps positioning during EMB.
    • 2DE enables targeted biopsies from specific ventricular regions.
    • 2DE is a valuable tool for predicting successful tissue sampling in EMB procedures.
