Summary

A low percentage of direction errors on the anti-saccade task after the first methylphenidate (MPH) dose in adults with attention-deficit/hyperactivity disorder (ADHD) may predict remission. MPH improved motor planning and response inhibition.

Background:

  • Adults with attention-deficit/hyperactivity disorder (ADHD) often exhibit deficits in motor planning and response inhibition.
  • The anti-saccade task (AST) is a sensitive measure of these executive functions.

Purpose of the Study:

  • To investigate if anti-saccade task (AST) performance after an initial methylphenidate (MPH) dose predicts clinical outcomes in adults with ADHD.
  • To assess the impact of MPH on AST performance in ADHD.

Main Methods:

  • Ninety-seven drug-naive adults with ADHD and 50 healthy controls (HCs) underwent AST testing at baseline, after a single MPH dose (10 mg), and after 6 months of chronic MPH treatment.
  • Multivariate regression analysis was used to identify predictors of remission.

Main Results:

  • ADHD adults initially demonstrated impaired AST performance (longer reaction times, more errors) compared to HCs.
  • Both acute and chronic MPH administration normalized AST performance in ADHD adults.
  • A low percentage of direction errors (≤10%) after the first MPH dose independently predicted remission at 6 months (OR: 5.84, p=0.001).

Conclusions:

  • Methylphenidate (MPH) effectively improves motor planning and response inhibition deficits in adults with ADHD.
  • Early AST performance following a single MPH dose can serve as a predictive biomarker for treatment response in ADHD.
