Background:

  • Plaque psoriasis involves epidermal thickening, complicating topical drug delivery.
  • Tacrolimus (TAC) is an immunosuppressant used for psoriasis but faces solubility challenges.

Purpose of the Study:

  • To develop and evaluate tacrolimus (TAC) nanocrystals (NCs) dispersed in a microneedle patch (MNP) for enhanced psoriasis treatment.
  • To assess the efficacy of TAC NCs MNP in overcoming delivery barriers in psoriatic skin.

Main Methods:

  • TAC NCs were prepared using a bottom-up technique and incorporated into sodium hyaluronate-based MNPs.
  • Microneedle mechanical strength, insertion depth, and drug retention were quantified.
  • Pharmacodynamic studies evaluated the therapeutic effects on psoriatic skin features and inflammatory markers.

Main Results:

  • TAC NCs had an average particle size of 259.6 ± 2.3 nm.
  • MNPs demonstrated sufficient mechanical strength for psoriatic skin penetration (depth 258.8 ± 14.4 μm).
  • Intradermal retention of TAC NCs MNP was six times higher than commercial ointment, with significant improvements in skin features and reduced TNF-α, IL-17A, and IL-23 levels.

Conclusions:

  • TAC NCs MNP effectively penetrates thickened psoriatic epidermis.
  • The MNP formulation significantly enhances drug retention and therapeutic efficacy compared to conventional treatments.
  • TAC NCs MNP represents a promising advanced drug delivery strategy for plaque psoriasis management.
