Upadacitinib treatment in rheumatoid arthritis patients led to significant patient-reported remission by week 60, as measured by the Routine Assessment of Patient Index Data 3 (RAPID3) tool. These findings were consistent across multiple clinical trials.
Area of Science:
- Rheumatology
- Clinical Trials
- Patient-Reported Outcomes
Background:
- The Routine Assessment of Patient Index Data 3 (RAPID3) is a validated patient-reported outcome measure for assessing rheumatoid arthritis (RA) disease activity.
- Upadacitinib is an approved treatment for RA, and its long-term efficacy is continually evaluated.
Purpose of the Study:
- To evaluate the long-term effect of upadacitinib on RAPID3 scores in patients with RA.
- To assess the proportion of patients achieving RAPID3 remission with upadacitinib treatment over 60 weeks.
Main Methods:
- Post hoc analysis of five phase 3 randomized controlled trials (RCTs) within the SELECT clinical trial program.
- Patients received upadacitinib 15 mg or 30 mg once daily, as monotherapy or with csDMARDs.
- RAPID3 remission (score ≤ 3) and correlations with CDAI, SDAI, and DAS28(CRP) were assessed at week 60.
Main Results:
- 3117 patients were included across five RCTs.
- By week 60, 32-52% of methotrexate-naïve and csDMARD-IR patients achieved RAPID3 remission with upadacitinib.
- RAPID3 scores demonstrated strong correlations with CDAI, SDAI, and DAS28(CRP) (Spearman's ρ ≥ 0.58, p < 0.001).
Conclusions:
- Upadacitinib monotherapy or combination therapy is associated with sustained patient-reported remission in RA patients over 60 weeks.
- RAPID3 is a reliable tool for assessing treatment response in RA patients receiving upadacitinib.

Assessment of apical radial pulse
The A-R pulse assessment involves simultaneous evaluation of the apical and radial pulses. When the apical and radial pulse rates vary, this assessment helps identify a pulse deficit.
