Community Based Intervention01:30

Community Based Intervention

88
Community-based interventions in mental health represent a paradigm shift from institution-centered care to treatments embedded within the fabric of local communities. By prioritizing inclusion and leveraging existing societal structures, this approach fosters a supportive environment conducive to addressing mental health challenges while promoting individual dignity and agency.

Specialized Care Centers and Settings-I

990
Specialized care settings or centers are situated in convenient locations within the community and offer care to a specific group or population. They consist of daycare facilities, mental health facilities, rural health facilities, educational institutions, industries, shelters for the homeless, and rehabilitation facilities.

Rural Health Centers
Rural health centers are specialized care facilities in remote locations with very few medical personnel. The primary care providers who run the centers are mostly Registered Nurse Practitioners. Here, emergency treatment is provided to critically ill or injured patients before they are transferred to the closest hospital. Fortunately, due to advancement in technology, many rural healthcare facilities and professionals have easy access to diagnostic and treatment...

Amnesia01:13

Amnesia

182
Amnesia is a condition marked by long-term memory loss, which impairs the ability to recall past events or create new memories.
The severity and duration of memory loss vary depending on the type and underlying cause. Amnesia is classified into two main types: retrograde and anterograde.
Retrograde amnesia is marked by the loss of memories formed before the onset of the condition. Patients may recall distant past events but often forget those occurring shortly before the incident.
Anterograde...

Restorative Care01:19

Restorative Care

2.0K
Restorative care is provided once a patient has been discharged from a healthcare facility and requires additional services. The additional services include home care, rehabilitation programs, and extended care. Restorative care centers help the patient regain their previous level of functioning or acquire a new level of functioning due to the incapacitating effects of a disease or a disability. It aims to assist patients in enhancing their quality of life by encouraging independence,...

False Memories01:18

False Memories

135
False memories represent a cognitive distortion in which individuals recall events that did not happen, or remember them in an altered form. This phenomenon highlights the brain's constructive nature in processing and recalling memories, emphasizing that memory is not a perfect representation of past events but rather a dynamic reconstruction influenced by various factors.
One primary source of false memories is misattribution, where individuals incorrectly associate external information...

Summary

Implementing a new general practitioner-led model improved dementia detection in regional areas. However, a significant number of patients indicated for referral declined, highlighting a need for improved dementia care pathways.

Area of Science:

  • Geriatrics
  • Public Health
  • Primary Care Medicine

Background:

  • Dementia presents a significant global health challenge, impacting patients, families, and society.
  • Early dementia detection and intervention improve patient outcomes and quality of care.
  • Screening for dementia is notably limited in rural and regional areas, despite a high and growing burden.

Purpose of the Study:

  • To implement and evaluate a novel general practitioner (GP)-led, multidisciplinary model of care for dementia detection.
  • To establish a referral pathway to a community-based specialist clinic for individuals in regional settings.
  • To address the gap in dementia screening and care within rural and regional communities.

Main Methods:

  • A cross-sectional analysis was conducted across six regional general practices.
  • Demographic data and medical history were collected.
  • Cognitive function was assessed using the Mini-Mental State Examination (MMSE), GP Assessment of Cognition (GPCog), and Geriatric Depression Scale (GDS).
  • Referral data and outcomes to geriatrician services were recorded.

Main Results:

  • 818 patients aged 65 and over were screened, representing 24.2% of eligible presentations.
  • 68.9% of screened patients were indicated for referral, but only 30.3% were successfully referred.
  • 34.2% of referred patients declined the referral, with many showing intermediate cognitive assessment scores.

Conclusions:

  • Standardized models of care integrated with community services are essential for enhancing dementia detection, referral, and management.
  • The developed model provides valuable insights for future service development, particularly for rural and regional dementia care.
  • Addressing barriers to referral acceptance is crucial for improving dementia care access in underserved populations.
