Background:

  • The growing elderly population necessitates improved communication skills training for transitional care.
  • Current transitional care communication skills training (CST) is insufficient for the aging demographic.

Purpose of the Study:

  • To evaluate the effectiveness of an online transitional care CST (OTCCST) intervention combined with transitional care (TC).
  • To assess the impact of the OTCCST and TC intervention from the perspectives of healthcare providers (HCPs), older patients, and family members.

Main Methods:

  • Randomized controlled trial involving 38 HCPs and 84 patient-family pairs.
  • Primary outcome: HCP communication confidence. Secondary outcomes: patient quality of life (QoL), activities of daily living (ADL), rehospitalization rates, and caregiver burden.
  • Data collected via validated questionnaires and rehospitalization tracking for 3 months post-discharge; analyzed using multiple regression.

Main Results:

  • HCPs in the experimental group demonstrated significantly improved communication confidence compared to the control group (p=0.0006).
  • Patients receiving the intervention had significantly fewer rehospitalizations within 3 months post-discharge (p<0.05).
  • No significant differences were observed in patient QoL, ADL, or family caregiver burden between groups.

Conclusions:

  • The OTCCST effectively enhances HCP communication confidence.
  • Combining OTCCST with TC reduces rehospitalization rates in older patients.
  • OTCCST provides a flexible, asynchronous learning format suitable for continuing professional development.
