The two-point peak-trough method for vancomycin AUC monitoring is effective in older Chinese patients. This approach, targeting AUC/MIC ≥ 400, showed a higher excellent response rate compared to trough-only monitoring.
Area of Science:
- Pharmacology
- Clinical Pharmacy
- Infectious Diseases
Background:
- Vancomycin area under the curve/minimum inhibitory concentration (AUC/MIC) is a proposed therapeutic drug monitoring (TDM) target.
- Traditional AUC estimation is time-consuming; a two-point trough-peak method offers a simpler alternative.
- The two-point AUC/MIC method and target have not been validated in Chinese patients.
Purpose of the Study:
- To compare clinical outcomes of vancomycin therapy in Chinese older adults (>60 years).
- To evaluate trough-only versus two-point peak-trough AUC TDM approaches for vancomycin dosing.
Main Methods:
- Patients were divided into study (two-point TDM, target AUC/MIC ≥ 400) and control (trough-based TDM, target trough 15-20 mg/L) groups.
- Vancomycin effectiveness assessed via clinical findings, lab values, and bacteriologic response.
- Treatment effects and kidney function compared between groups.
Main Results:
- 200 patients included: 80 in the study group, 120 in the control group. Average age was 69.8 years.
- Staphylococcus aureus was the most common pathogen (34%).
- Excellent vancomycin response rates were significantly higher in the study group (90%) vs. control group (73.3%), P=0.0039. No vancomycin-related nephrotoxicity observed.
Conclusions:
- The two-point peak-trough method is a practical approach for vancomycin AUC estimation.
- An AUC/MIC ≥ 400 target is effective and safe for vancomycin therapy in older Chinese patients.
