    Background:

    • Complete hydatidiform mole (CHM) is a pregnancy complication with potential for malignant transformation.
    • Maternal immunization against fetal antigens, particularly Human Leukocyte Antigen (HLA) systems, is a known phenomenon.

    Purpose of the Study:

    • To investigate the role of HLA and ABO blood group systems in the immune response to CHM.
    • To determine if maternal-fetal ABO incompatibility influences the risk of developing post-mole trophoblastic tumors.

    Main Methods:

    • Analysis of HLA and ABO blood group compatibility in patients with CHM.
    • Monitoring antibody detection rates against HLA-ABC antigens at different stages (evacuation vs. treatment).

    Main Results:

    • CHM can induce immunization against paternally derived HLA-ABC antigens.
    • Higher frequency of antibody detection (10/14) was observed at the time of treatment for trophoblastic tumors compared to evacuation (2/14).
    • Patients with ABO blood group incompatibility between mother and husband had a significantly higher incidence of requiring treatment for post-mole trophoblastic tumors (17/57) compared to those with ABO compatibility (0/25).

    Conclusions:

    • The stroma of placental villi is a likely source of immunogen in CHM.
    • ABO blood group incompatibility between the maternal environment and molar tissue appears to favor trophoblastic proliferation, increasing the risk of post-mole tumors.
