Confocal Fluorescence Microscopy01:16

Confocal Fluorescence Microscopy

Confocal microscopy is an advanced microscopic technique. The prime advantage of the confocal microscope over other microscopy techniques is its ability to block the out-of-focus light from the illuminated samples using pinholes. It is widely used with fluorescence optics to obtain high-resolution, sharp contrast images. Unlike optical microscopes, confocal microscopes use a focused beam of light laser to scan the entire sample surface at different z-planes. These microscopes are, therefore,...

Summary

This study evaluated a new femtosecond laser system for refractive corneal lenticule extraction, finding it safe and predictable. While most patients achieved good vision, those requiring extra surgical steps had slower visual recovery.

Area of Science:

  • Ophthalmology
  • Laser Surgery
  • Corneal Refractive Surgery

Background:

  • Refractive errors are common vision impairments.
  • Femtosecond lasers offer precise corneal tissue ablation.
  • New technologies aim to improve refractive surgery outcomes.

Purpose of the Study:

  • To assess the Ziemer CLEAR femtosecond laser system for refractive corneal lenticule extraction.
  • To evaluate its suction control, centration, and alignment capabilities.
  • To determine the refractive and visual outcomes of the procedure.

Main Methods:

  • Retrospective, consecutive case series of 53 eyes from 42 patients.
  • Patients with spherical equivalent (SE) between -3 and -10 D underwent the procedure.
  • Outcomes evaluated included SE, visual acuity, and centration at 10 months.

Main Results:

  • All lenticules were extracted successfully without suction loss.
  • At 10 months, mean uncorrected visual acuity was 0.05 logMAR, with 70% achieving 20/25 or better.
  • Mean SE was -0.13 D, with 90% achieving refractive astigmatism ≤0.50 D and high efficacy (1.00) and safety (0.98) indices.

Conclusions:

  • The Ziemer CLEAR femtosecond laser system demonstrates good predictability, efficacy, and safety for refractive lenticule extraction.
  • Slower visual recovery was noted in eyes requiring additional surgical manipulations.
  • The system's computerized regulation of centration and alignment contributes to successful outcomes.
