Background:

  • Conventional MRI has limited value in diagnosing thyroid nodules.
  • The diagnostic utility of multi-parameter diffusion-weighted imaging (DWI) for thyroid nodules is not well-established.
  • Quantitative evaluation of thyroid nodules using advanced DWI techniques is needed.

Purpose of the Study:

  • To assess the effectiveness of multi-parametric DWI models (mono-exponential, bi-exponential, stretched exponential, kurtosis) in differentiating thyroid lesions.
  • To determine the diagnostic performance of various DWI-derived parameters for thyroid nodule characterization.

Main Methods:

  • Seventy-nine patients with thyroid nodules underwent multi-b value DWI.
  • Analysis included mono-exponential, bi-exponential, stretched exponential, and kurtosis DWI models.
  • Statistical analysis compared diffusion parameters between benign and malignant nodules.

Main Results:

  • Malignant thyroid nodules showed significantly lower diffusion parameters (ADC, D, f, DDC, α, Dapp) and higher kurtosis (Kapp) compared to benign nodules (P < 0.01).

  • Multiple DWI parameters (ADC, D, f, DDC, α, Dapp, Kapp) can effectively discriminate between benign and malignant thyroid nodules.
  • Metrics like D, DDC, Dapp, and Kapp offer complementary information to ADC, potentially enhancing diagnostic accuracy when combined.
  • Increasing b values for ADC calculation may not necessarily improve diagnostic performance for thyroid nodules.
