Cystic Fibrosis: Management01:24

Cystic Fibrosis: Management

214
Cystic fibrosis (CF) is an autosomal recessive disorder that predominantly affects individuals of Northern European descent, occurring at a rate of 1 in 3500. It is caused by a genetic mutation in a gene on chromosome 7, most commonly the ΔF508 mutation, that codes for the cystic fibrosis transmembrane conductance regulator (CFTR) protein. This results in thicker mucus secretions and obstruction pathologies in multiple organs, including the lungs and sinuses.

Inflammatory Bowel Disease V: Surgical Management

195
Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Area of Science:

  • Otolaryngology
  • Paediatric Surgery

Background:

  • Cholesteatoma is more aggressive in children than adults, presenting complex management challenges.
  • Optimizing surgical outcomes and monitoring requires adapting to new technologies.

Approach:

  • Review of current technologic advancements and practice adaptations in paediatric cholesteatoma management.
  • Evaluation of endoscopic ear surgery, chemical-assisted dissection, and advanced imaging protocols.

Key Points:

  • Endoscopic ear surgery offers viable management options with comparable or improved outcomes.
  • Chemical-assisted dissection, using agents like Mesna, aids in reducing cholesteatoma recurrence.
  • Advanced imaging, particularly diffusion-weighted MRI, enhances surgical precision and patient monitoring.

Conclusions:

  • Modern otologic surgery leverages technological progress for superior paediatric cholesteatoma care.
  • Mesna shows promise in improving recidivism rates for paediatric cholesteatoma.
  • Diffusion-weighted MRI protocols are crucial for effective management of this challenging paediatric condition.
