Summary

This study introduces a novel totally endoscopic aortic valve replacement technique. Early results in 266 patients show low mortality and acceptable morbidity, indicating a promising minimally invasive approach.

Background:

  • Aortic valve replacement is a critical procedure for severe aortic valve stenosis.
  • Traditional open-heart surgery carries significant risks and prolonged recovery.
  • Minimally invasive techniques are sought to improve patient outcomes and reduce complications.

Purpose of the Study:

  • To describe a new technique for totally endoscopic aortic valve replacement (TEAVR).
  • To evaluate the early outcomes of TEAVR in a consecutive patient cohort.

Main Methods:

  • Retrospective analysis of 266 consecutive patients undergoing TEAVR between October 2017 and December 2020.
  • Exclusion of reoperations and combined procedures.
  • Median follow-up of 0.69 years.

Main Results:

  • 250 patients (93.98%) underwent TEAVR for aortic valve stenosis.
  • 30-day major adverse cardiac and cerebrovascular events occurred in 1.50% of patients.
  • Overall hospital mortality was 1.50%, with 7.52% mortality at 3-year follow-up.
  • Early thoracoscopic revision was needed in 7 patients; 14 required permanent pacemaker implantation.

Conclusions:

  • Totally endoscopic aortic valve replacement demonstrates satisfactory early results in a large patient cohort.
  • The technique is associated with low mortality and acceptable morbidity rates.
  • TEAVR represents a viable minimally invasive alternative for aortic valve replacement.
