Schedule exercise therapy (SET) effectively treats chronic insomnia disorder (CID). Combining SET with medication significantly improved sleep quality and reduced insomnia symptoms compared to medication alone.
Area of Science:
- Sleep Medicine
- Behavioral Therapy
Background:
- Chronic insomnia disorder (CID) significantly impacts quality of life.
- Nonpharmacological interventions are crucial for managing CID.
Purpose of the Study:
- To evaluate the efficacy of Schedule Exercise Therapy (SET) as a novel nonpharmacological treatment for CID.
- To compare the effects of medication alone (MED) versus medication combined with SET (MSET) on sleep and mood in CID patients.
Main Methods:
- 118 CID patients were randomized into MED or MSET groups.
- Sleep and mood status were assessed over 12 weeks using PSQI, ISI, ESS, SDS, and SAS.
- Clinical effectiveness and hypnotic use rates were calculated at study endpoint.
Main Results:
- Both groups showed significant improvements in sleep quality and mood (PSQI, ISI, ESS, SDS, SAS).
- The MSET group demonstrated significantly greater improvements in PSQI, ISI, SDS, and SAS compared to the MED group.
- MSET resulted in a higher clinical effective rate and lower hypnotic usage rate than MED.
Conclusions:
- Schedule Exercise Therapy (SET) is a promising and effective treatment for chronic insomnia disorder.
- Combining SET with medication enhances treatment outcomes for CID patients, improving sleep and mood while reducing reliance on hypnotics.

Melatonin congeners like ramelteon (Rozerem) and tasimelteon (Hetlioz) selectively bind to melatonin receptors (MT1 and MT2) and thus mimic the actions of melatonin, a hormone that regulates sleep-wake cycles. Tasimelteon is primarily used for non-24-hour sleep-wake disorder, common in blind patients. They are also used to treat conditions like insomnia...
