    Summary

Midwives rarely obtain informed consent for episiotomy during labor due to timing and fear of refusal. Providing information earlier in pregnancy or via a birth plan could improve consent practices for this surgical procedure.

    Area of Science:

    • Obstetrics and Gynecology
    • Maternal Healthcare
    • Surgical Procedures in Obstetrics

    Background:

    • Despite increased patient demand for birth participation, informed consent for episiotomy remains a challenge.
    • Previous studies indicated improved information but rarely sought consent for episiotomy.
    • This study investigates current midwife practices regarding episiotomy information and consent.

    Purpose of the Study:

    • To explore midwives' practices in informing women about episiotomy and obtaining consent.
    • To identify factors influencing midwives' ability to inform and gain consent in the delivery room.
    • To assess changes in practice since previous research.

    Main Methods:

    • Conducted 12 semi-structured interviews with delivery room midwives in the Ile-de-France region.
    • Performed manual content analysis independently by two researchers.
    • Utilized NVivo software for complementary analysis to validate findings.

    Main Results:

    • Information and consent collection for episiotomy were limited by the anxiety-provoking nature of the topic and the perceived inappropriate timing during labor.
    • Midwives expressed fear of refusal as a barrier to seeking consent.
    • Pre-pregnancy information and birth plans were identified as facilitators for consent.

    Conclusions:

    • Dedicated time for informed consent discussions regarding episiotomy is crucial.
    • Distributing patient information leaflets on episiotomy could enhance professional practices.
    • Improving informed consent practices supports women's autonomy in childbirth decisions.
