Abstract

Insights

Current guidelines lack specific strategies to prevent multidrug-resistant Gram-negative bacilli (MDR-GNB) surgical site infections (SSIs). Evidence suggests surgical antibiotic prophylaxis with aminoglycoside may reduce MDR-GNB SSIs in renal transplant recipients, but more research is needed.

Area of Science:

  • Infectious Diseases
  • Surgical Infections
  • Antimicrobial Resistance

Background:

  • Surgical site infections (SSIs) caused by multidrug-resistant Gram-negative bacilli (MDR-GNB) lack specific preventive recommendations.
  • This systematic review evaluates measures to prevent MDR-GNB SSIs.

Approach:

  • Searched MEDLINE, EMBASE, CINAHL, and LILACS databases up to February 18, 2020.
  • Included randomized trials and observational cohort studies on preventive measures for MDR-GNB SSIs in adult surgical patients.
  • Evaluated study quality using Newcastle-Ottawa, ROBINS-I, and GRADE methods; performed random-effects meta-analyses.

Key Points:

  • Identified 10,663 titles; included 3 studies (2 retrospective, 1 prospective).
  • Meta-analysis of 2 renal transplantation studies (854 patients) showed a relative risk of 0.57 (95%CI: 0.25-1.34) for MDR-GNB SSI with surgical antibiotic prophylaxis (SAP) including aminoglycoside (GRADE: moderate).
  • One study evaluated ertapenem vs. cephalosporin plus metronidazole for SAP in extended-spectrum beta-lactamase producing Enterobacteriales carriers undergoing colon surgery.

Conclusions:

  • Insufficient data currently support specific measures against MDR-GNB SSIs.
  • Prospective, randomized studies are essential to confirm the efficacy and safety of SAP with aminoglycoside for MDR-GNB SSI prevention in renal transplant recipients and other patient groups.

Healthcare Associated Infections II: Preventive Measures

Essential infection prevention measures are based on the knowledge of the infection chain, the modes of transmission in healthcare settings, and the use of the best practices in all healthcare settings. Compulsory public reporting of healthcare-associated infection rates is needed to allow individuals and the community to make informed choices regarding selecting a healthcare facility.
The best practices for preventing healthcare-associated infections include hand hygiene, patient risk...

Antimicrobial Effectiveness01:28

Antimicrobial Effectiveness

The effectiveness of antimicrobial agents depends on various factors influencing their ability to eliminate microbial populations. Larger microbial populations require more time for complete eradication, emphasizing the importance of population size analysis when evaluating antimicrobial efficacy.Microbial resistance to antimicrobial agents varies significantly. Highly resilient microorganisms include endospores, gram-negative bacteria, and non-enveloped viruses, while prions are exceptionally...

Hand hygiene01:23

Hand hygiene

Asepsis is the practice of preventing or breaking the chain of infection. The nurse employs aseptic techniques to prevent the spread of microorganisms and reduce the risk of diseases. Hand hygiene is the cornerstone of aseptic techniques and is classified into medical and surgical asepsis. Medical asepsis includes hand hygiene and the use of gloves. Surgical asepsis, or the sterile technique, refers to practices that render and keep objects and areas free of microorganisms.

Combined Effects of Drugs: Synergism01:27

Combined Effects of Drugs: Synergism

Synergism is a useful mechanism where combining two or more drugs is more effective than each constituent used alone. Such combinations are also called supra-additive interactions. The drugs collectively enhance the final therapeutic effect by acting on different targets. Another advantage is that the low dose of each constituent drug is sufficient to achieve the desired effect. This helps reduce the duration of therapy and lower the adverse effects of these drugs.
Such synergistic combinations...

Factors Affecting the Risk of Infection01:26

Factors Affecting the Risk of Infection

The hosts' susceptibility to infection depends on several factors. The integrity of the skin and mucous membranes helps protect the body against microbial attacks. When the skin is altered, the chance of infection, limb loss, and even death increases.
The integrity and count of the white blood cells help the body resist pathogens and fight infection. When impaired, it reduces the body's resistance to pathogens. The acidic pH levels of the gastrointestinal, genitourinary tracts, and skin...

Acute Pyelonephritis II: Diagnostic Studies and Management

Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...
