[Status of radiologic pelvimetry].

    Summary

    Radiological pelvimetry has controversial diagnostic value. This study suggests limiting its routine use, recommending it only for specific cases of breech presentation or post-Caesarean section when disproportionate head-pelvis ratios are suspected.

    Background:

    • Radiological pelvimetry is used to assess pelvic dimensions in pregnant women.
    • Its diagnostic value, particularly in breech presentation and post-Caesarean section, remains debated.
    • Current practices involve its use at the Department of Gynaecology in Aarau.

    Purpose of the Study:

    • To evaluate the prospective diagnostic value of radiological pelvimetry.
    • To assess the incidence of abnormal pelvic shapes and narrow pelvic dimensions.
    • To determine the utility of radiological pelvimetry in cases of breech presentation and after Caesarean section.

    Main Methods:

    • Analysis of 164 radiological pelvimetric measurements in breech presentation.
    • Review of 112 X-ray films from patients post-Caesarean section.
    • Comparison of radiological findings with digital palpation assessments.

    Main Results:

    • Abnormal pelvic shapes and narrow pelvic dimensions were found with similar frequency.
    • Approximately 20% of cases in both breech presentation and post-partum groups indicated a disproportionate head-pelvis relationship.
    • Radiologically narrowed pelvic dimensions were identified in 10% of women; funnel-shaped pelvis was most common.
    • Pathologically narrowed pelves and generally narrow pelves were detectable by digital palpation.
    • All cases with conjugata vera below 10 cm involved generally narrow pelves, detectable by palpation.

    Conclusions:

    • Routine radiological pelvimetry is not recommended due to its limited additional diagnostic value.
    • Digital palpation effectively identifies most pathologically narrowed pelves and narrow pelvic dimensions.
    • Lateral pelvic X-ray may assist in specific difficult birth scenarios, such as breech presentation or suspected disproportion post-Caesarean section.
