    Area of Science:

    • Orthopedic Surgery
    • Podiatry
    • Biomechanical Engineering

    Background:

    • Hallux valgus, commonly known as a bunion, is a prevalent foot deformity.
    • Surgical interventions are often considered for symptomatic hallux valgus when conservative treatments fail.
    • The Keller procedure is one surgical option for addressing hallux valgus, particularly in older populations.

    Purpose of the Study:

    • To prospectively evaluate the efficacy of the Keller procedure for treating hallux valgus.
    • To assess the impact of the surgery on patient symptoms, function, cosmesis, and deformity.
    • To determine the overall patient satisfaction rate following the Keller procedure.

    Main Methods:

    • A prospective study involving 44 female patients (75 feet) with hallux valgus.
    • Average patient age was 66 years.
    • Follow-up duration averaged 31 months (range: 12-64 months).

    Main Results:

    • The Keller procedure significantly improved symptoms like joint pain and bunion tenderness.
    • Functional outcomes, including footwear and activity levels, showed improvement.
    • A decrease in the hallux valgus deformity was observed.
    • Metatarsal calluses were not influenced by the procedure.
    • Postoperative cock-up deformity was common but did not negatively impact results.
    • Overall patient satisfaction was 77%.

    Conclusions:

    • The Keller procedure is an effective surgical option for hallux valgus in older female patients.
    • The surgery leads to significant improvements in pain, function, and cosmetic appearance.
    • Despite common postoperative deformities, patient satisfaction is generally high, indicating a favorable risk-benefit profile.
