Summary

Radiation-associated sarcomas (RAS) are aggressive. Malignant peripheral nerve sheath tumors and undifferentiated pleomorphic sarcomas show higher disease-specific death, with radiation association independently increasing MPNST risk.

Background:

  • Radiation-associated sarcomas (RAS) are rare but aggressive malignancies.
  • Understanding histology-specific behavior is crucial for prognostication.

Purpose of the Study:

  • To characterize the histology-specific presentation and behavior of soft tissue RAS.
  • To improve individualized prognostication for RAS patients.

Main Methods:

  • Querying a prospectively maintained database of primary, nonmetastatic RAS treated with surgical resection (1982-2019).
  • Propensity-matching patients with common RAS histologies to those with sporadic tumors of the same histology.
  • Modeling disease-specific death (DSD) using cumulative incidence analyses.

Main Results:

  • Among 259 RAS patients, the most common histologies were MPNST, myxofibrosarcoma, leiomyosarcoma, UPS, and AS.
  • DSD varied significantly by histology, with RAS MPNST and UPS showing the highest rates.
  • RAS MPNST and UPS had higher DSD compared to sporadic counterparts; RAS AS, leiomyosarcoma, and myxofibrosarcoma showed similar DSD. After matching, only RAS MPNST demonstrated a significant difference in DSD compared to sporadic MPNST.

Conclusions:

  • RAS presentation and DSD are histology-specific, primarily driven by RAS MPNST and UPS.
  • Standard prognostic factors are applicable for most RAS histologies.
  • Radiation association independently increases the risk of DSD in MPNST.
