A rare pigmented variant of multinucleate cell angiohistiocytoma (MCAH) was identified. This benign skin condition features peculiar multinucleate cells containing melanin pigment.

Background:

  • Multinucleate cell angiohistiocytoma (MCAH) is a benign vascular and fibrohistiocytic proliferation.
  • Its characteristic multinucleate cells are typically interpreted as degenerated macrophages.
  • Clinical variants exist, some presenting with brownish lesions.

Observation:

  • A case of pigmented MCAH in a 33-year-old woman is presented.
  • Multiple papules and plaques were observed on the right elbow, jawline, and left flank.
  • Cytoplasmic brownish pigment was noted within the multinucleate cells.

Findings:

  • The pigment stained positive with Masson-Fontana stain.
  • The pigment was negative with iron staining.
  • Histological findings are consistent with melanin deposition in the multinucleate cells.

Implications:

  • This case expands the known histological spectrum of MCAH.
  • It highlights the presence of melanin within the multinucleate cells of MCAH.
  • Further research may clarify the origin and significance of melanin in this condition.
