Background:

  • Hydroceles are common scrotal swellings, but spontaneous rupture is a rare complication.
  • Information regarding the clinical course and management of ruptured hydroceles is scarce in medical literature.

Observation:

  • A 64-year-old male presented with a spontaneous, atraumatic left hydrocele rupture.
  • The hydrocele had been diagnosed as non-complex and ruptured three years after its initial onset.
  • The patient reported no pain following the rupture.

Findings:

  • Post-rupture, mild fluid re-accumulation was observed during clinical follow-up.
  • While fluid re-accumulation was minimal in this patient, significant re-accumulation is possible based on existing reports.

Implications:

  • This case highlights the potential for spontaneous hydrocele rupture and its generally benign post-rupture course.
  • Further case reports are needed to better understand the management and prognosis of ruptured hydroceles.
