Area of Science:

  • Cardiovascular Surgery
  • Biomaterials Science
  • Medical Device Engineering

Background:

  • Aortic root and left ventricular outflow tract (LVOT) replacement often requires complex surgical solutions.
  • Conventional methods may limit the size of prosthetic valves that can be implanted.
  • Redo-aortic root replacement presents unique challenges, especially with LVOT restrictions.

Purpose of the Study:

  • To describe a novel self-assembled composite graft modification for aortic root and LVOT replacement.
  • To enable the implantation of a larger bioprosthesis than typically possible.
  • To facilitate simultaneous reconstruction of the LVOT during aortic root surgery.

Main Methods:

  • A modified self-assembled composite graft technique was developed.
  • The technique involves the use of an Inspiris Resilia aortic valve and a Gelweave Valsalva graft.
  • The bioprosthesis is positioned within the sinus portion of the graft to accommodate a slightly smaller valve.

Main Results:

  • The described technique allows for the implantation of a larger aortic valve.
  • Simultaneous reconstruction of the LVOT is achieved.
  • The method provides a suitable collar length for LVOT reconstruction.

Conclusions:

  • This novel graft modification offers a viable solution for aortic root and LVOT replacement.
  • It is particularly advantageous for patients undergoing redo-aortic root surgery with LVOT restrictions.
  • The technique facilitates the use of larger prosthetic valves in complex cases.
