    Background:

    • Postpartum fever requires thorough investigation, especially in foreign nationals.
    • Tuberculosis (TB) is a global health concern, with varied presentations.
    • Immigrant populations may have different risk factors and disease patterns.

    Observation:

    • Two foreign national women presented with prolonged, unexplained postpartum fever.
    • Extensive diagnostic work-up was required to identify the underlying cause.
    • Pulmonary tuberculosis was diagnosed in both cases.

    Findings:

    • Pulmonary tuberculosis was identified as the cause of postpartum fever in these cases.
    • Antitubercular drugs led to a favorable response and resolution of symptoms.
    • The presentation of TB can be atypical in the postpartum period.

    Implications:

    • Physicians should consider tuberculosis in the differential diagnosis of postpartum fever in immigrant patients.
    • Awareness of changing TB patterns and diagnostic challenges is essential.
    • Timely diagnosis and treatment of TB in this population can improve outcomes.
