Background:

  • Nodular regenerative hyperplasia (NRH) is a rare liver condition causing noncirrhotic portal hypertension.

  • A 29-year-old male with X-linked agammaglobulinemia and NRH presented with pancytopenia and elevated liver enzymes.
  • Imaging revealed numerous hepatic lesions, and biopsy confirmed hepatic angiosarcoma.

Findings:

  • The patient had advanced hepatic angiosarcoma and was ineligible for treatment.
  • Autopsy confirmed widespread angiosarcoma infiltrating the liver.

Implications:

  • This case suggests a potential association between NRH and the development of malignancy, specifically angiosarcoma.
  • Further research is warranted to explore the link between NRH, immunodeficiency, and cancer risk.
