Background:

  • Deep surgical site infection (SSI) is a significant complication following distal femur fracture fixation.
  • Identifying modifiable risk factors is crucial for improving patient outcomes and reducing healthcare costs.

Purpose of the Study:

  • To identify potentially modifiable risk factors associated with deep surgical site infection (SSI) after surgical fixation of distal femur fractures.

Main Methods:

  • A multicenter retrospective cohort study was conducted involving 1107 patients with OTA/AO 33A or C distal femur fractures.
  • Data on patient demographics, injury characteristics, surgical procedures, and outcomes were collected from ten Level-I trauma centers.
  • Multivariate analysis was used to identify independent predictors of deep SSI.

Main Results:

  • The overall rate of deep SSI was 7% (79/1107).
  • Independent risk factors for deep SSI included alcohol abuse (OR = 2.36), intra-articular injury (OR = 1.73), and vascular injury (OR = 3.90).
  • Increased surgical duration (OR = 1.15 per hour) was also a risk factor, while the use of topical antibiotics showed a protective effect (OR = 0.50).

Conclusions:

  • Alcohol abuse, intra-articular involvement, vascular injury, and longer surgical duration are significant risk factors for deep SSI after distal femur fracture fixation.
  • The use of topical antibiotics may reduce the risk of deep SSI.
  • These findings highlight the importance of addressing modifiable risk factors to prevent SSIs in this patient population.
