    Summary

    Diffuse panbronchiolitis (DPB) is a distinct condition from bronchiolitis obliterans. Morphological analysis revealed intraluminal granulation tissue and thickened respiratory bronchiole walls as key features in DPB.

    • To elucidate the distinct morphological characteristics of diffuse panbronchiolitis.
    • To classify the changes observed in the respiratory bronchioles.

    Main Methods:

    • Serial sectioning and reconstruction of lesions from 20 diffuse panbronchiolitis cases.
    • Focus on the region from terminal to respiratory bronchioles.

    Main Results:

    • Respiratory bronchiolar walls showed thickening due to cell infiltration and granulation tissue.
    • Intraluminal granulation tissue was present in 26 of 28 foci.
    • Xanthoma cells were observed distally to stenotic sites and in alveolar spaces.
