Skin Cancer01:30

Skin Cancer

4.4K
Skin cancer is a type of cancer that occurs when there is an abnormal growth of skin cells, usually triggered by damage to the DNA within the skin cells. It is primarily caused by exposure to ultraviolet (UV) radiation from the sun or artificial sources like tanning beds. Skin cancer is the most common type of cancer worldwide, and its incidence continues to rise.

Papillary Dermis

3.2K
Dermis
The dermis might be considered the "core" of the integumentary system, as distinct from the epidermis and hypodermis. It contains blood and lymph vessels, nerves, and other structures, such as hair follicles and sweat glands. The dermis is made of two layers of connective tissue that comprise an interconnected mesh of elastin and collagenous fibers, produced by fibroblasts.
Papillary Layer
The papillary layer is made of loose, areolar connective tissue, which means the collagen...

    Summary

Dermatofibrosarcoma protuberans (DFSP) management requires wide surgical excision and complex reconstruction. This study highlights successful outcomes in seven DFSP patients, emphasizing multidisciplinary care for this rare skin cancer.

    Area of Science:

    • Oncology
    • Dermatology
    • Surgical Pathology

    Background:

    • Dermatofibrosarcoma protuberans (DFSP) is a rare, low-grade, non-melanoma skin cancer.
    • Wide surgical excision with adequate margins is the primary treatment for DFSP.
    • Reconstruction of large post-excision defects often necessitates advanced surgical techniques.

    Purpose of the Study:

    • To report the clinical experience and management of seven consecutive patients with DFSP.
    • To evaluate the effectiveness of surgical excision and reconstruction in DFSP cases.
    • To emphasize the importance of a multidisciplinary approach in managing DFSP.

    Main Methods:

    • Retrospective study of seven DFSP patients from July 2018 to July 2022.
    • All patients underwent wide local excision or re-excision.
    • Reconstruction involved split-thickness skin grafts (STSG) and local flaps.

    Main Results:

    • Mean patient age was 38.2 years.
    • Four patients had primary excision, three required re-excision.
    • Complex reconstructions were necessary for all cases, including STSG and local flaps.
    • Two cases showed fibrosarcomatous differentiation and received adjuvant radiotherapy.

    Conclusions:

    • DFSP management, especially for larger tumors, necessitates incisional biopsy and a multidisciplinary approach.
    • Wide surgical excision achieving negative histopathological margins is the standard treatment.
    • DFSP is a low-grade sarcoma with rare metastatic potential, emphasizing the importance of complete surgical removal.
