Insights
Intrauterine growth restriction (IUGR) can lead to lifelong cardiovascular problems, including hypertension. Research highlights mechanisms of fetal programming for cardiovascular disease (CVD) in IUGR.
Area of Science:
- Cardiovascular Science
- Developmental Biology
- Perinatal Medicine
Background:
- Intrauterine growth restriction (IUGR) affects 5-15% of pregnancies, impacting neonatal growth and long-term health.
- IUGR is linked to hypertension, renin-angiotensin system (RAS) activation, and endothelial dysfunction.
- These factors contribute to cardiovascular diseases (CVD) throughout life.
Purpose of the Study:
- To review new findings on cardiovascular dysfunction in IUGR.
- To outline mechanisms of fetal programming for hypertension and CVD in IUGR.
- To highlight experimental findings demonstrating CVD in IUGR.
Main Methods:
- Review of epidemiological and experimental studies.
- Focus on recent research investigating IUGR and CVD.
- Analysis of mechanisms mediating hypertension and cardiovascular dysfunction.
Main Results:
- IUGR is associated with significant cardiovascular dysfunction, including hypertension.
- Disruptions in placental signaling (mTORC, TGFβ) and RAS activation are key mechanisms.
- Evidence demonstrates the development of CVD in IUGR fetuses, children, and adults.
Conclusions:
- IUGR has profound, long-lasting effects on cardiovascular health.
- Understanding these mechanisms is crucial for developing therapeutic interventions.
- Further experimental studies are needed to address long-term CVD risks in IUGR individuals.
Purpose Of Review:
We highlight important new findings on cardiovascular dysfunction in intrauterine growth restriction.
Recent Findings:
Intrauterine growth restriction (IUGR) is a multifactorial condition which negatively impacts neonatal growth during pregnancy and is associated with health problems during the lifespan. It affects 5-15% of all pregnancies in the USA and Europe with varying percentages in developing countries. Epidemiological studies have reported that IUGR is associated with the pathogenesis of hypertension, activation of the renin-angiotensin system (RAS), disruption in placental-mTORC and TGFβ signaling cascades, and endothelial dysfunction in IUGR fetuses, children, adolescents, and adults resulting in the development of cardiovascular diseases (CVD). Experimental studies are needed to investigate therapeutic measures to treat increased blood pressure (BP) and long-term CVD problems in people affected by IUGR. We outline the mechanisms mediating fetal programming of hypertension in developing CVD. We have reviewed findings from different experimental models focusing on recent studies that demonstrate CVD in IUGR.

Fetal Circulation
Two umbilical arteries transport blood from the fetus to the placenta. At the placenta, the blood absorbs oxygen and nutrients while simultaneously eliminating waste products. This oxygen-enriched and nutrient-rich blood then returns to the fetus through one...

Regulation of the Cardiovascular System
The regulation of the cardiovascular system involves the autonomic nervous system (ANS), baroreceptors, and chemoreceptors, ensuring that heart rate and blood pressure are appropriately modulated in response to varying physiological demands.
The ANS comprises two main divisions: the sympathetic and parasympathetic nervous systems. The sympathetic nervous system enhances...
