    Background:

    • Spasticity is a common and disabling symptom in multiple sclerosis (MS).
    • Current treatments for MS-related spasticity have limitations, necessitating the exploration of new therapeutic agents.

    Purpose of the Study:

    • To evaluate the efficacy and safety of progabide, a gamma-aminobutyric acid agonist, in treating spasticity associated with multiple sclerosis.

    Main Methods:

    • A randomized, double-blinded, placebo-controlled crossover trial involving 32 patients with MS-related spasticity.
    • Patients received progabide (up to 45 mg/kg) or placebo for two four-week periods, separated by a two-week washout.
    • Outcomes assessed included spasticity reduction, motor power, and adverse events.

    Main Results:

    • Progabide demonstrated a significant reduction in spasticity without causing increased motor weakness.
    • Physicians, patients, and the study coordinator showed a preference for progabide.
    • Forty percent of participants opted to continue progabide in a long-term follow-up.
    • Seven serious adverse events were reported, including elevated liver enzymes, all of which resolved upon drug discontinuation.

    Conclusions:

    • Progabide is an effective agent for managing spasticity in multiple sclerosis.
    • The observed efficacy is not compromised by increased motor impairment.
    • A high incidence of adverse events may restrict the long-term therapeutic application of progabide.
