Topical vancomycin hydrochloride effectively treated severe Staphylococcus epidermidis blepharoconjunctivitis. Alternative preparations in saline or buffered artificial tears reduced ocular irritation while maintaining antimicrobial efficacy.

    Area of Science:

    • Ophthalmology
    • Microbiology
    • Pharmacology

    Background:

    • Severe blepharoconjunctivitis caused by Staphylococcus epidermidis requires effective treatment.
    • Topical vancomycin hydrochloride is a potential therapeutic option for resistant ocular infections.

    Observation:

    • Two patients with severe Staphylococcus epidermidis blepharoconjunctivitis were treated with a 50 mg/ml topical vancomycin hydrochloride solution prepared with sterile water.
    • This aqueous preparation caused significant ocular irritation.

    Findings:

    • Solutions of vancomycin hydrochloride (5 mg/ml and 50 mg/ml) prepared with normal saline or phosphate-buffered artificial tears demonstrated equivalent in vitro antimicrobial activity.
    • These alternative preparations were significantly less irritating compared to the sterile water-based solution.

    Related Experiment Videos

    Implications:

    • Vancomycin hydrochloride eye drops can be formulated in less irritating vehicles like normal saline or buffered artificial tears.
    • These findings suggest improved tolerability for topical vancomycin in treating ocular infections.
    • Topical vancomycin should be reserved for cases where standard antibiotics are ineffective.
