Background:

Transgender women (TW) retain their prostate post-surgery, requiring prostate cancer (CaP) screening.
Prevalence of transgender identification is increasing, yet CaP screening in TW is understudied.

Approach:

A narrative review of PubMed literature up to June 2022, including case reports, was conducted.
The review focused on prostate-specific antigen (PSA) screening practices and CaP incidence in TW.

Key Points:

Major medical societies lack consensus on PSA screening for TW; guidelines are absent.
Available data suggest CaP in TW may be more aggressive or castrate-resistant.
Further research is crucial to understand CaP risks and screening benefits in TW.

Conclusions:

Understanding of CaP screening in TW is in its early stages.
Future research should address screening risks/benefits, psychological impacts, and establish baseline PSA values for those on gender-affirming hormone therapy (GAHT).
Establishing screening initiation timing and biomarker accuracy for TW on GAHT is essential.
