Association between oxygen delivery and digital ulcers in systemic sclerosis

Reduced oxygen delivery (DO2) is lower in systemic sclerosis (SSc) patients than controls, but does not correlate with digital ulcers (DUs). Microvasculopathy remains the primary cause of DUs in SSc.

Purpose of the Study:
- To investigate the association between oxygen delivery (DO2) and the presence of digital ulcers (DUs) in patients with systemic sclerosis (SSc).

- Calculated DO2 using cardiac output, arterial oxygen saturation (SpO2), and hemoglobin levels in 111 SSc patients and 30 controls.
- DO2 was adjusted for weight and body surface area (BSA), utilizing both index-finger SpO2 and maximum SpO2 values.

- Mean DO2 was significantly lower in SSc patients compared to controls when adjusted for BSA (p=0.03).
- No significant difference in DO2 was observed between SSc patients with and without DUs (p>0.05).
- Patients with DUs exhibited higher modified Rodnan skin score (mRSS), lower forced vital capacity (FVC), and more frequent diffuse disease, ILD, and anti-SCL70 positivity.
Conclusions:
- While SSc patients exhibit lower DO2 than controls, DO2 levels do not differ between those with and without DUs.
- These findings suggest that DO2 plays a negligible role in DU development.
- The study reinforces the primary role of microvasculopathy in the pathogenesis of DUs in SSc.
