Background:

  • Laparoscopic colorectal surgery is increasing globally.
  • The need for second laparoscopic colorectal resection (SLCR) may rise.
  • Experience with SLCR is limited.

Purpose of the Study:

  • To evaluate the safety and feasibility of SLCR.
  • To compare SLCR outcomes with first elective colorectal resections.

Main Methods:

  • Retrospective analysis of a prospectively collected database (2008-2020).
  • Analysis of demographics, operative variables, and postoperative outcomes.
  • Propensity score matching with a control group undergoing first resection.

Main Results:

  • 32 patients (1.7%) underwent SLCR.
  • Median operative time: 170 minutes; 9% intraoperative complications, 6% conversions.
  • Postoperative morbidity: 34% overall, 19% major; 12.5% reoperation rate; 3.1% mortality.
  • SLCR more frequently performed by colorectal surgeons; no significant differences in perioperative outcomes compared to controls.

Conclusions:

  • SLCR can be safely performed without compromising perioperative outcomes.
  • Further research is needed to confirm benefits of minimally invasive approach for colorectal second resection.
  • Long-term outcomes of SLCR require further investigation.
