MicroRNA (miRNA) are short, regulatory RNA transcribed from introns (non-coding regions of a gene) or intergenic regions (stretches of DNA present between genes). Several processing steps are required to form biologically active, mature miRNA. The initial transcript, called primary miRNA (pri-mRNA), base-pairs with itself, forming a stem-loop structure. Background:

  • Hand osteoarthritis (OA) is a prevalent musculoskeletal condition, particularly in aging populations.
  • Identifying reliable biomarkers for hand OA is crucial for diagnosis and management.

Purpose of the Study:

  • To investigate circulating microRNAs (miRNAs) as potential biomarkers in plasma of hand OA patients.
  • To compare miRNA profiles in patients with hand OA against age- and sex-matched healthy controls.

Main Methods:

  • Plasma samples from 238 participants (96 erosive hand OA, 73 non-erosive hand OA, 69 controls) were analyzed.
  • MicroRNA screening utilized TaqMan™ Low-Density Array, with validation via quantitative real-time PCR (qRT-PCR).
  • Clinical assessments included AUSCAN, Algofunctional index, and Kallman radiographic scoring.

Main Results:

  • Forty miRNAs showed differential expression between hand OA patients and controls in the initial screening.
  • Three miRNAs (miR-23a-3p, miR-146a-5p, miR-652-3p) were significantly increased in hand OA patients.
  • Elevated miRNAs correlated with AUSCAN scores and pain; miR-222-3p inversely correlated with radiographic severity.

Conclusions:

  • Circulating miRNA profiles offer potential candidate biomarkers for hand OA symptoms.
  • Further longitudinal studies are necessary to elucidate the precise role of these miRNAs in hand OA pathogenesis.
