The Thyroid Gland01:23

The Thyroid Gland

4.1K
The thyroid gland is a small, butterfly-shaped gland located in the neck and covers the anterior surface of the trachea. The gland has two lateral lobes connected by a thin tissue mass called the isthmus. Internally, each lobe comprises many small spherical structures known as thyroid follicles, surrounded by a network of blood vessels.
The follicles have a central cavity lined by simple cuboidal to squamous epithelial cells called follicular cells. These cells produce the glycoprotein...

Synthesis and Regulation of Thyroid Hormones01:20

Synthesis and Regulation of Thyroid Hormones

4.9K
Low blood levels of the thyroid hormones — triiodothyronine (T3) and thyroxine (T4) — signal the hypothalamus to release the thyrotropin-releasing hormone (TRH). TRH then reaches the pituitary gland and stimulates the release of thyroid-stimulating hormone(TSH) into the bloodstream.
Upon reaching the thyroid gland, TSH stimulates the follicular cells' active uptake of iodide ions from the blood. The ions diffuse to the apical surface of the cells and are oxidized to iodine. The...

Functions of Thyroid Hormones01:18

Functions of Thyroid Hormones

3.0K
The thyroid hormone (TH) plays a pivotal role in the intricate orchestration of physiological processes, exerting profound effects on development, metabolism, and homeostasis throughout different life stages.
TH is indispensable for the normal development and maturation of the skeletal, muscular, and nervous systems during fetal and childhood growth. It facilitates bone mineral turnover and regulates protein synthesis in developing tissues, contributing significantly to overall growth and...

Acute Pyelonephritis II: Diagnostic Studies and Management

30
Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

Endoscopic Studies II: Thoracocentesis

436
Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...

Area of Science:

  • Endocrinology
  • Infectious Diseases
  • Surgical Pathology

Background:

  • Suppurative thyroid gland infections leading to abscess formation are uncommon due to the thyroid's inherent protective characteristics.
  • This case highlights a rare instance of thyroid abscess in a young female patient with co-existing pulmonary tuberculosis and Klebsiella pneumonia.

Observation:

  • The patient presented with a rapidly enlarging, tender thyroid gland causing significant respiratory distress.
  • Clinical examination and radiological imaging confirmed the presence of a thyroid abscess.

Findings:

  • Surgical incision and drainage of the thyroid abscess were performed.
  • Appropriate antibiotic therapy was administered concurrently with surgical management.

Implications:

  • A rapidly enlarging thyroid gland in patients with active bacterial infections warrants high suspicion for thyroid abscess.
  • Early diagnosis and prompt, aggressive management, including surgical drainage and antibiotics, are critical for favorable outcomes and can be life-saving.
