Primary hepatic angiosarcoma (PHA) is a rare liver cancer. Contrast-enhanced ultrasound (CEUS) shows characteristic liver nodules and aids biopsy, improving diagnosis of this challenging vascular tumor.

Background:

Primary hepatic angiosarcoma (PHA) is a rare vascular endothelial malignancy.
Clinical presentation and standard diagnostics for PHA often lack specificity.
This case highlights diagnostic challenges and imaging features of PHA.

Observation:

A 75-year-old woman presented with nonspecific abdominal pain and multiple liver lesions on initial imaging.
Contrast-enhanced ultrasound (CEUS) revealed suspicious hyperechoic nodules.
CT and MRI showed nonspecific findings, mimicking other liver pathologies.

Findings:

Laboratory results included decreased platelets and elevated D-dimer.
Histopathology confirmed primary hepatic angiosarcoma.
