Corneal edema associated with degenerating Soemmering ring cataract: Clinical-pathologic correlation

Background:

• Cataract extraction is a common procedure, but delayed complications can arise.
• Soemmering ring cataracts, a remnant of lens material, can calcify over time.

Observation:

• Three patients (75-92 years old) presented with corneal edema 20-30 years post-cataract surgery.
• Calcific lens particles from degenerated Soemmering ring cataracts dispersed into the anterior chamber and vitreous.
• Deposits on the posterior cornea caused localized edema and endothelial cell loss; one patient developed secondary glaucoma.

Findings:

• Histopathology confirmed extracellular calcific deposits on Descemet's membrane, associated with corneal endothelial cell loss.
• Acellular material similar to deposits was found in aqueous and vitreous samples.
• Treatments included endothelial keratoplasty, anterior chamber lavage, and Soemmering ring cataract removal, with improved outcomes in patients undergoing keratoplasty.

Implications:

• Dispersion of calcific Soemmering ring cataracts is a rare but significant delayed complication.
• This phenomenon can lead to corneal edema, secondary glaucoma, and vitreous opacities.
• Prompt diagnosis and management are essential to prevent irreversible ocular morbidity.
