Summary

Multiple vestibular disorders, like Vestibular Migraine (VM), Benign Paroxysmal Positional Vertigo (BPPV), and Meniere Disease (MD), significantly increase the risk of developing Persistent Postural-Perceptual Dizziness (PPPD). VM is a key factor in precipitating PPPD.

Background:

  • Benign Paroxysmal Positional Vertigo (BPPV), Vestibular Migraine (VM), and Meniere Disease (MD) are common episodic vestibular disorders.
  • Persistent Postural-Perceptual Dizziness (PPPD) is a chronic functional vestibular disorder that can develop following these conditions.

Purpose of the Study:

  • To investigate the role of single or multiple vestibular comorbidities as precipitating factors for PPPD.
  • To analyze the incidence and risk associated with BPPV, VM, and MD in the development of PPPD.

Main Methods:

  • Evaluation of 376 patients with a history of vestibular disorders.
  • Detailed anamnesis to assess diagnostic criteria for PPPD.
  • Statistical analysis of comorbidity prevalence and precipitating conditions.

Main Results:

  • PPPD was diagnosed in 6.4% of patients (24 out of 376).
  • Incidence of PPPD was 3.9% with single vestibular comorbidities and 22.4% with multiple comorbidities.
  • Vestibular Migraine (VM) was a significant precipitating condition for PPPD, both alone and with other disorders. BPPV was a precipitating factor mainly in the first episode.

Conclusions:

  • The presence of multiple vestibular comorbidities substantially increases the risk of developing PPPD.
  • Vestibular Migraine plays a crucial role in precipitating PPPD, highlighting the importance of managing migraine in dizzy patients.
