Bones of the Upper Limb: Radius

The radius is longer of the two bones that make up the human antebrachium or forearm. At the proximal end, the radius articulates with the capitulum of the humerus and the radial notch of the ulna to form the elbow joint. At the distal end, the radius articulates with the ulna via the ulnar notch, forming the distal radioulnar joint. Distally, the radius also attaches to the carpal wrist bones (scaphoid and lunate) to form the radiocarpal joint.

Summary

Complex dislocations of the metacarpophalangeal joint, also known as Kaplan’s Lesion, are rare hand injuries. This case highlights successful surgical treatment using a dorsal approach for irreducible dislocation.

Background:

Complex dislocation of the metacarpophalangeal joint (MCPJ) is an uncommon hand injury.
Also referred to as Kaplan's Lesion, these injuries are categorized as simple/reducible or complex/irreducible.
Surgical intervention strategies for MCPJ dislocations vary.

Observation:

A 36-year-old weightlifter presented with an irreducible complex dislocation of the metacarpophalangeal joint.
The patient's injury was successfully managed through a dorsal surgical approach.

Findings:

The dorsal approach provided effective treatment for this rare complex metacarpophalangeal joint dislocation.
Successful management highlights the viability of the dorsal approach in specific cases of Kaplan's Lesion.

Implications:

This case underscores the importance of recognizing and appropriately managing complex MCPJ dislocations.
The successful dorsal approach offers a valuable treatment option for irreducible Kaplan's Lesions.
Further discussion on etiopathogenesis and management strategies for these injuries is warranted.
