Background:

  • Abdominal tuberculosis (ATB) is an uncommon manifestation of tuberculosis (TB), affecting the gastrointestinal tract, peritoneum, lymph nodes, and solid organs.
  • Prevalence of ATB is higher in individuals from TB-endemic regions, necessitating awareness of epidemiological risk factors.

Observation:

  • A 47-year-old man from India presented with diarrhea, abdominal pain, anorexia, asthenia, and weight loss.
  • Imaging revealed ileal and mesenteric thickening with lymphadenopathies; colonoscopy showed cecal and ileal lesions.
  • Initial treatment for suspected colitis/IBD was ineffective, delaying ATB diagnosis.

Findings:

  • Histopathology of cervical lymphadenopathy showed granulomatous lymphadenitis with necrosis, prompting antituberculostatic therapy.
  • Despite initial improvement, a cavitated lung nodule and Mycobacterium tuberculosis complex in bronchoalveolar lavage were identified.
  • Delayed diagnosis and treatment contributed to the complex presentation.

Implications:

  • Timely diagnosis and adequate treatment are essential for reducing mortality in intestinal tuberculosis.
  • Epidemiological risk factors are critical for prompt diagnosis and must be considered in all suspected cases.
  • This case highlights the importance of considering ATB in patients with relevant risk factors presenting with gastrointestinal symptoms.
