Summary

Surgical margins did not impact overall survival in extremity soft tissue sarcoma (eSTS) patients. Achieving negative margins may not be necessary if it requires limb amputation or significant functional impairment.

Area of Science:

  • Orthopedic Oncology
  • Surgical Oncology
  • Sarcoma Research

Context:

  • Extremity soft tissue sarcomas (eSTS) are rare tumors.
  • Surgical resection is a primary treatment modality for eSTS.
  • The impact of surgical margin status on patient outcomes is crucial for treatment planning.

Purpose:

  • To evaluate the effect of surgical margins on local recurrence (LR), distant recurrence (DR), and overall survival (OS) in patients with eSTS.
  • To identify prognostic factors influencing survival in eSTS.

Summary:

  • A cohort of 128 primary eSTS patients treated between 2006-2010 was analyzed.
  • Positive surgical margins (R1 resection) were observed in 22.7% of cases.
  • Local recurrence occurred in 27%, distant recurrence in 13%, with a 5-year OS of 84%.

  • Clinical stage, tumor type, and histological grade were significant prognostic factors for OS.
  • Surgical margin status did not significantly impact overall survival.
  • Impact:

    • Findings suggest that aggressive surgical techniques to achieve negative margins, potentially leading to limb amputation or functional deficits, may not improve overall survival in eSTS.
    • Treatment decisions should balance oncological principles with functional preservation.
    • Further research may explore risk stratification for margin management in eSTS.
